Admissions at Pinecrest Manor
Our Admissions Director is available to answer any questions you may have regarding the admissions process, and you are encouraged to schedule a tour of the facility as well as the surrounding garden areas. We realize this can be a complex and sometimes uncomfortable process and we will work with you to ensure a smooth transition to our facility.
Admissions may occur from the hospital, home or another facility. If you will be admitting directly from the hospital, the social worker at the hospital can submit referral information, which will be evaluated by the admissions coordinator to ensure the facility can meet our individual needs. If admitting from home or another facility, information can be obtained from your family doctor.

- History and physical
- Demographics information
- Insurance information
- Medication list
- Treatment notes
- Physical or occupational therapy notes
